• We are looking for an enthusiastic individual to join our hospital team on a vital role in managing safe and efficient hospital discharges. This role is set in a fast-paced environment where delivering results and meeting performance targets is essential.
  
  
  

Key Responsibilities:

• Multi-Disciplinary Assessments: Specialising in multi-disciplinary assessments with health professionals, including DSTs.
• Duty Management: Overseeing daily discharges via the D2A pathway and Pathway 3 placements. Triaging cases in collaboration with the Hillingdon HUB and managing out-of-borough hospital referrals. Attending conference calls with health professionals and ensuring effective decision-making.
• Staff Supervision: Allocating cases, managing stepdown beds, and supervising staff to ensure smooth team operations.
• Safeguarding Management: Attending weekly meetings with the safeguarding lead to track the progression of investigations.
• Case Management: Carrying a small caseload of complex cases, providing expertise and oversight where needed.
  
  
  

Requirements:

The post holder must have completed a Practice Educator Training Qualification.
