Purpose of job The Post holder will be an integral part of the Occupational Therapy & Sensory Services. Delivering high quality services to support adults using a person centred approach. They will focus on well-being, prevention, choice & control for individuals, maximising their independence / potential. The Occupational Therapist will meet with Service Users, carers, families, multi-disciplinary agencies and organisations (in a variety of locations as required) in order to enhance the independence, safety and quality of life of the Service User and their carer (s) through rehabilitative planning, specification of equipment and structural changes to the environment; producing individual care and support plans, strategies, monitoring and case management, this will include coordinating assessments and reports, including presentations to relevant multi-agency panels. They will ensure that all work complies with Council policies / procedures and that statutory obligation are met. Occupational Therapists are required to adhere to the College of Occupational Therapy Code of Ethics & Professional Conduct and Health & Care Professions Council Standards of Proficiency within their role. To be responsible to the Advanced Practitioner for: The quality and prioritisation of own work and for alerting Line Management to any workload management issues. The adoption of own good practice and performance standards, adherence to Directorate policies, procedures, statutory responsibilities and agreed performance targets. Main duties of the post
  • To assess the needs of Service users, in an appropriate environment, formulating care & support plans, making arrangements for service provision, rehabilitative guidance and advice, minor and major adaptations, standard and specialist equipment and information / advice.
  • To participate in multi professional meetings /Panel & Forums as required
  • To undertake assessments in line with legislation, local and national policies, procedures, guidelines and eligibility criteria.
  • To make arrangements for the provision, installation and demonstration of equipment and adaptations. Ensuring an up-to- date knowledge of specialist equipment, adaptations and services offered by Occupational Therapists in the community.
  • To identify, alert and refer any complex and/or safeguarding issue (s] on to the appropriate specialist social work team as soon as possible.
  • To manage a caseload, commensurate with grade and experience, identifying and reducing / managing risks, exploring the most effective method of intervention and coordinating the provision and review of equipment / adaptations / moving and handling advice, as appropriate
  • To undertake the assessment of carers.
  • To work in partnership with colleagues in other council agencies and primary / secondary and tertiary organisations to provide services to people in need of Occupational Therapy.
  • To create and maintain accurate and contemporaneous service user records, as required by relevant policies and procedures, good practice guidelines and professional bodies.
  • To undertake Moving and Handling risk assessments and provide appropriate equipment and training to service users and carers.
  • To manage conflict, address complex and contentious issues and find ways to resolve them with service users, carers or colleagues in a professional manner.
  • To maintain own competence to practice through continuing professional development and contribute to the development of the service as a whole.
  • To undertake any other work appropriate to the level and general nature of the posts duties, including service, staff and team development.
  • To undertake all duties with due regard to the provision of Health and Safety regulations and legislation, the councils equal opportunities and customer care policies, and the new technology agreement.
  • To be accountable to the Team Manager for the supervision of, and allocation of work to, up to 2 staff.
  • To maintain a professional portfolio, to meet requirements of Continued Professional Development/ the Health Care Professions Council requirement
  • To participate in the transition process for young people with disabilities through to adulthood, in liaison with other departments and agencies
  • To comply with financial, personnel, administrative, management information, clients records and systems as directed by Team Manager.
  • To participate in Duty/Intake/ Enquiry Systems as required and to be responsible.
  • for prioritising incoming work to the service in consultation with managers as
  • appropriate.
  • To respond to, and collate information relating to complaints and service
  • provision issues as requested by the OT line management
