- About the role
- As a Social Worker, you will play a key role in improving mental health outcomes for Hillingdon residents. You will work in close collaboration with residents, families, carers, and multidisciplinary teams to provide person-centred, recovery-focused support.
As part of your day-to-day responsibilities, you will be expected to:
- Complete Care Act Assessments
- Undertake Mental Capacity Act assessments
- Make placement requests and manage the Reablement / Discharge to Assess (D2A) process
- Handle complex cases, including housing referrals and homelessness applications
- Work in partnership with health professionals such as Hospitals and CCGs
- Attend and contribute to multi-disciplinary team meetings, family meetings, and best interest meetings
- Carry out Section 42 safeguarding investigations
- Provide supervision to both newly qualified and unqualified practitioners within the team
- Undertake Practice Educator training, where relevant
