About the Role:

This is an excellent opportunity for two passionate and dedicated individuals to work collaboratively with our young people. You will play a crucial role in supporting and empowering young people currently in care, as well as those who have left care and are eligible for leaving care services. Through practical and planned interventions, you will help them make a successful transition to adulthood.

Working with care leavers can be a very rewarding role in being able to support and help a young person make change in their lives and continue their current pathway to independent living.

At times our care leavers require a little more support, which might see a personal advisor attend meetings for education, housing, benefits, health, or general catch up for a chat. As a personal advisor you will sign post a young person for services.

You will be required to work in the office at least 3 days per week.

Core hours are 09:00am - 17:00pm, although this is dependent upon when a young person can meet with you. Young people live in Southwark, surrounding areas, out of London, are in University and can be out of the area due to risk. Your visits will therefore see a change to your core hours.

You require knowledge and skills in working with looked after children and care leavers.

RESPONSIBILITIES

1. Work in partnership with the young adults, carers, and other professionals to

assess the young adults’ needs and undertake actions to address them.

2. Manage the caseload as directed and organise and plan work activities

prioritising tasks and responsibilities.

3. Adapt to a fast-paced environment and challenges that the profession can bring.

3. Provide written and verbal reports, which are concise, informative, and based on

analysis of complex evidence.

4. Contribute to the drawing up, implementing and reviewing of the young adults’

pathway plan with the young adults’ participation.

5. Work with key professionals from the department and externally to offer advice

and implement pathway plans.

6. Ensure pathway plans are up to date and reviewed in line with the statutory

requirements.

7. Attend and contribute to young adults’ meetings and any adult safeguarding

meetings, including Court as appropriate.

8. Undertake the statutory role of personal advisor as defined in Care leavers’

regulations 2010, planning transitions to adulthood 2011, and the Children’s Act

1989 and Children and Social Work Act 2017.

9. Develop and maintain close working relationships with key agencies to ensure a

co-ordinated response to young adult.

10. Work individually and within groups of young adults as required, by undertaking

regular visits as per the statutory requirement.

11. Maintain working knowledge of issues that impact on young adults in

care\leaving care including benefits, exploitation, parenting, housing, and training

and employment opportunities.

12. Keep a sound recording of the various communications with young adults as

well as professionals.

13. Produce written reports, maintain records and data as required

14. Be involved in department, service and practice group development and

training opportunities.

2 JOB DESCRIPTION – PERSONAL ADVISOR

15. Ensure all work undertaken takes into account the diverse needs of care leaver

and assist young adults which can sometimes be at short notice and respond to

their issue within a reasonable period or as an emergency matter.

16. Prepare for and use supervision actively.

17. Prepare for case supervision and being able to meet agreed deadline.
