This is an excellent opportunity for an experienced support professional to provide practical and person-centred support to carers of individuals with learning disabilities and neurodiverse needs. Working within a community-focused service, you will manage a caseload of carers, provide tailored advice and guidance, facilitate peer support activities, and connect individuals with the services and resources they need to thrive.

Please note this role will be subject to an enhanced DBS check due to unsupervised contact with vulnerable adults.

Key Responsibilities:

  • Manage referrals and undertake assessments to identify the support needs of carers.
  • Provide one-to-one advice, guidance, signposting and ongoing casework support.
  • Develop and implement support plans to help carers achieve positive outcomes.
  • Build effective relationships with health, social care, education and community-based services to support carers.
  • Facilitate and develop peer support groups, forums and activities that reduce isolation and encourage peer networks.
  • Promote the service at meetings, events and through partnership working to increase awareness and engagement.
  • Maintain accurate case records, outcome data and monitoring information using CRM systems.
  • Respond appropriately to safeguarding concerns and ensure all work is delivered in line with relevant legislation and best practice.

Key Requirements:

  • Experience working within health and social care, mental health, education, family support, community services or a related environment.
  • Experience providing one-to-one support and managing a varied caseload.
  • Understanding of learning disabilities, neurodiversity, SEND processes and statutory support pathways.
  • Strong assessment, advocacy and relationship-building skills.
  • Experience building partnerships and working collaboratively with multiple stakeholders.
  • Ability to work independently, manage competing priorities and thrive in a busy environment.
  • Confident IT skills, including the use of case management and CRM systems. CharityLog experience is highly desirable.
  • A compassionate, person-centred approach and a commitment to improving outcomes for carers and their families.

If you are passionate about helping people access the financial support they are entitled to and have the experience to navigate complex welfare cases, we would love to hear from you.
